Beer is the worst alcohol for gout due to brewer yeast purines on top of ethanol. Wine has fewer purines but its alcohol still impairs uric acid excretion. Distilled spirits are purine-free but the alcohol itself remains a gout risk. ACR 2020: avoid all alcohol during flares.
Alcohol worsens gout through two mechanisms: ethanol metabolism increases uric acid production, and its metabolite lactate competes with uric acid for kidney excretion. Beer adds a third mechanism — brewer's yeast purines. Epidemiological studies show beer drinkers have 2-3x higher gout risk than non-drinkers. Wine shows weaker but still present risk. Distilled spirits are purine-free but alcohol itself remains a significant factor.
The American College of Rheumatology recommends limiting all alcohol, and completely avoiding it during acute gout flares. No amount of alcohol is truly safe for gout — the question is about degree of risk, not elimination of risk.
